Pre-School Checklist Kindergarten - 8th Grade Checklist

Pre-School Application Checklist

  1. Tours are given by appointment from 9am - 11 am daily. You will be required to fill out a basic form at this time.
  2. An appointment for the parent interview and Student Evaluation will be set up at this time.
  3. Please set up a Registration appointment to return the application, applicable fees and required forms. The following will be needed.
  4. The application will be released upon your students acceptance into our program.

a. Birth Certificate
b. Medical Form
c. Health Insurance (proof of)
d. Permanent Record Request Form
e. Uniform Order Form
f. Enrollment Forms
g. Please bring credit/debit cards; money order, or Certified check.

Kindergarten - 8th Grade Application Checklist

  1. Tours are given by appointment from 9am - 12pm daily. You will be required to fill out a basic form at this time.
  2. Parents will be given a recommendation form to be filled by the administrator of the student's previous school.
  3. When we receive the recommendation by mail we will call you to schedule a testing evaluation date for your student.
  4. An appointment for the parent interview will be set up at that time & the test results will be discussed during the meeting.
  5. The application will be released upon your students acceptance into our program
